Incident Description:

A Pipeline Crew was tasked with installing and bolting up 30’’ pipeline sections. All pipe sections and elbows were resting on wooden skids.

Crew members were bolting up a flanged elbow while other crew members were working on the placement of a 120’ section of pipe; both tasks were taking place in close proximately to each other. As a worker was bolting up a flange via an impact gun, the 120’ section of pipe was rigged with a nylon sling and lifted via a side boom. The work area was on a slight incline, causing the 120’ section of pipe to swing and contact the elbowed flange that was being bolted up. The contact shifted the flanged elbow, tipping the skids and pinning a worker’s left ankle under the flange.
